Question 1

A contractor is performing a Subsystem Hazard Analysis on a robotic loading arm. The arm's controller uses a purchased commercial-off-the-shelf (COTS) motion-control board with firmware written by another vendor. The project engineer proposes leaving the board and its firmware out of the SSHA because the contractor did not design them and the vendor says the board is reliable. What should the safety professional conclude?

Show answer and source

Answer: C. The board and firmware must be included, because the analysis covers COTS items and software.

The standard says the SSHA analysis shall include COTS, GOTS, GFE, NDI, and software, so the purchased board and its firmware are in scope. Excluding either one because another party designed it or the vendor calls it reliable is not allowed.

“This analysis shall include Commercial-Off-the-Shelf (COTS), Government-Off-the-Shelf (GOTS), Government-Furnished Equipment (GFE), Non-Developmental Items (NDI), and software.”

Question 2

A contractor developing an automated cargo-handling vehicle reviews the interlock that prevents the lift arm from moving while personnel are in the load zone. Engineers state that inspecting the design drawings and reviewing the logic cannot establish whether the interlock actually reduces the risk to an acceptable level. Under the system safety verification task, what is required next?

Show answer and source

Answer: B. Specify and conduct tests to evaluate the overall effectiveness of the mitigation measure, and integrate them into the appropriate T&E plans

When analysis or inspection cannot determine whether risk mitigation is adequate, the standard requires tests to be specified and conducted, and the safety tests must be built into the system T&E plans. Accepting the design on inspection alone ignores the stated limitation of inspection.

Answer B rests on

MIL-STD-882E, DoD Standard Practice: System Safety (2012), PDF p. 92(opens ecfr.gov in a new tab)
“401.2.1 When analysis or inspection cannot determine the adequacy of risk mitigation measures, tests shall be specified and conducted to evaluate the overall effectiveness of the mitigation measures. Specific safety tests shall be integrated into appropriate system Test and Evaluation (T&E) plans, including verification and validation plans.”

Question 3

A prime contractor is drafting its Hazard Management Plan (HMP) for a new vehicle program. The safety lead proposes listing only the prime's own design reviews and tests in the schedule section. Software development and a subcontractor's braking-subsystem testing are scheduled in separate engineering study plans, and the lead argues they do not belong in the HMP because they are "someone else's schedules." Which response is consistent with the HMP milestone requirements?

Show answer and source

Answer: C. The HMP should identify the schedules for subsystem, component, and software activities that apply to hazard management, even though they are specified in other engineering studies and development efforts.

The standard requires the HMP to identify schedules for subsystem, component, and software activities applicable to hazard management even when they are specified in other studies or development efforts. Limiting the schedule to the prime's own activities would leave out those required items.

Answer C rests on

“c. Identify the schedules for subsystem, component, and software activities applicable to the hazard management activities but specified in other engineering studies and development efforts.”
