A company's written injury reporting procedure requires employees to report any injury within one hour of the shift ending, and the company awards a monthly bonus to crews with no reported injuries. Several employees say they avoid reporting minor injuries because they fear losing the bonus. Under the recordkeeping employee-involvement requirements, how should this procedure be judged?

Answer: C. It is not reasonable if it would deter or discourage a reasonable employee from accurately reporting an injury or illness
The standard says a reporting procedure is not reasonable if it would deter or discourage a reasonable employee from accurately reporting a workplace injury or illness. Having a written procedure alone does not make it compliant.
“You must establish a reasonable procedure for employees to report work-related injuries and illnesses promptly and accurately. A procedure is not reasonable if it would deter or discourage a reasonable employee from accurately reporting a workplace injury or illness;”
On January 30, a safety manager posted the certified OSHA 300A summary on the break-room bulletin board, where company notices are customarily posted. On April 15, during a walkthrough, she finds that several flyers for a company picnic and a blood drive have been pinned over most of the summary. What action is required?

Answer: D. Remove the flyers so the summary is fully visible, and keep it posted in that location through April 30
The standard requires the employer to ensure the posted summary is not covered by other material, so the flyers must be removed and the summary kept visible in the customary notice location. Leaving it covered, or removing it to the safety office, would not satisfy the posting requirement.
“You must post a copy of the annual summary in each establishment in a conspicuous place or places where notices to employees are customarily posted. You must ensure that the posted annual summary is not altered, defaced or covered by other material.”
