CSP question for October 20, 2026

The CSP question of the day for Tuesday, October 20, 2026, on incident investigation and management of change (PSM). Pick an answer to see the explanation and its source.

CSP Domain 2: Program Management

A refinery unit covered by PSM is being modified under an approved management of change (MOC). The change replaces a manual transfer valve with a remotely actuated one, which alters the steps operators follow during transfers. It also changes how a contract maintenance crew isolates and services the line. The project manager wants to start up the unit Friday and proposes a toolbox talk for operators the following week. He says the contractor crew's own employer will brief them later. Which response is consistent with the standard?

Show answer and source

Answer: C. Reject the plan; operators and the affected contract maintenance employees must be informed of and trained in the change before start-up of the affected part of the process.

Under the Occupational Safety and Health Administration (OSHA) process safety management (PSM) rule, 29 CFR 1910.119(l)(3), operators and the maintenance and contract employees whose tasks a change affects must be informed of and trained in it before start-up of the process or affected part. A toolbox talk next week or a later contractor briefing is too late. Training cannot depend on whether process safety information changed, and contract employees are covered.

On the job

A site superintendent at a gas treatment plant reviews an approved change that adds a new pressure transmitter and revises the control room startup checklist for a covered process. Operators and the instrument technicians from a contractor firm will work differently once it runs. The superintendent postpones restart until both groups have been informed of and trained in the change, since that must happen before start-up of the affected part of the process.
