A refinery's written management of change (MOC) procedure is being applied to several proposed jobs on a covered process unit. Which proposed job is a change that must go through the MOC procedure before it is carried out?
Show answer and source
Answer: A. Substituting a different process chemical formulation with a different composition for the one currently charged to the unit
The standard requires written MOC procedures for changes to process chemicals, technology, equipment and procedures, except replacements in kind. A different chemical formulation is a change to process chemicals. The identical pump, transmitter and gasket are like-for-like replacements, so they fall under the exception.
“The employer shall establish and implement written procedures to manage changes (except for “replacements in kind”) to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process.”
A refinery unit covered by PSM is being modified under an approved management of change (MOC). The change replaces a manual transfer valve with a remotely actuated one, which alters the steps operators follow during transfers. It also changes how a contract maintenance crew isolates and services the line. The project manager wants to start up the unit Friday and proposes a toolbox talk for operators the following week. He says the contractor crew's own employer will brief them later. Which response is consistent with the standard?

Answer: C. Reject the plan; operators and the affected contract maintenance employees must be informed of and trained in the change before start-up of the affected part of the process.
The standard requires that operating employees and maintenance and contract employees whose job tasks are affected by a change be informed and trained before start-up of the process or affected part. Delaying operator training or leaving contractors to their own employer is wrong, and the training duty does not depend on whether the process safety information changed.
“(3) Employees involved in operating a process and maintenance and contract employees whose job tasks will be affected by a change in the process shall be informed of, and trained in, the change prior to start-up of the process or affected part of the process.”
