Entry supervisor
The person who decides whether conditions are acceptable, authorizes entry into a permit space, oversees it and ends it.
Definition, in the source's words
29 CFR 1910.146(b)(opens ecfr.gov in a new tab)“Entry supervisor means the person (such as the employer, foreman, or crew chief) responsible for determining if acceptable entry conditions are present at a permit space where entry is planned, for authorizing entry and overseeing entry operations, and for terminating entry as required by this section.”
What it means
The entry supervisor signs the entry permit after checking that the tests, isolation and rescue arrangements listed on it are in place, and cancels the permit when the work is done or conditions change.
Example
Before a vault entry, the crew chief checks the atmospheric test results against the permit's acceptable conditions, confirms rescue is available, and signs the permit. When the job is finished, the crew chief cancels the permit and ends the entry.
